Beaufort Lodge Rehabilitation Centre is going be a purpose built facility due to open in Kingston-upon-Thames in Autumn 2017. CHD Living is seeking experienced and highly motivated Assistant Practitioners to work with its multi-disciplinary team to implement an established care pathway in our new centre. The Centre will rehabilitate a mix of clients, from different funding pathways, all with varying rehab potential. The case load will be varied, requiring excellent rehab planning and time management skills.  

The successful applicants will work under the supervision of a Registered Practitioner (Doctors, Nurses, and Therapists) and will be able to perform the roles of an Assistant Practitioner within the rehabilitation & nursing community.

Job Purpose

  1. To ensure quality care enabling patients to be supported throughout their pathway whilst promoting independence and protecting choice, dignity, privacy and safety.
  2. To work in support of qualified staff and allied health care professionals in the assessment, planning, implementing and evaluation of care without direct supervision of a registered practitioner.
  3. To work as an Assistant Practitioner in ensuring that holistic care is delivered and standards of care are enhanced

  • Educated to NVQ 6 Level, Foundation Degree plus additional training to Degree level equivalent, or equivalent through short courses and relevant applied experience
  •  Experience in the speciality areas of stroke/neuro rehabilitation and post-operative orthopaedic care. Post-surgical management experience is highly desirable. 
  •  Demonstrable previous training and competencies in venepuncture, management/maintenance of PEG tubes, medication management, wound and catheter care.

SKILLS AND KNOWLEDGE:

  • Knowledge of nursing procedures and practices; such as enteral feeding, wound care (dressings), venepuncture, previous experience of assisting medical practitioners in minor intervention such as joint injections, nerve blocks etc.  
  • Significant experience 
  • Knowledge of Rehabilitation

  •  Experience of working in the rehabilitation setting

3. NURSES

We are looking for caring people who are passionate about supporting our residents to live their life to the full at CHD Living Care Centres.  You will be responsible for the wellbeing of residents within the home, ensuring that their, physical, emotional and social needs are met, whilst helping them to achieve their full capabilities.

You must have a genuine interest in providing high quality, person centred care.  You must be registered with the NMC, have excellent communication skills, be motivated and professional.

Main duties include:

  • Clinical assessments
  • Control and administration of medication
  • Hands on delivery of person centred care.

Experienced Physiotherapists – Band 5 & Band 6

Unit: Bagshot Park Centre for Neurorehabilitation

Pay range:  £24,799 – £32,765 / annum

Bagshot Park Neurorehabilitation Centre specialises in neurological rehabilitation and complex disability management, being well resourced and equipped to allow an expert multi-disciplinary team to provide an optimum rehabilitation service.

The Centre has a mix of clients from different funding pathways all with varying rehab potential. The case load will be varied, requiring excellent rehab planning, communication and time management skills.

We are seeking an experienced and highly motivated Senior Physiotherapist and a Band 6 Physiotherapist who would deliver positive outcomes as part of a high performing multi-disciplinary team and have a true passion for neurological rehabilitation.

They will work with the multi-disciplinary team to implement an established pathway within the centre. This is an excellent opportunity to develop your skills in rehabilitation using the centres fantastic resources.

The successful candidate will receive regular support from the Rehab Manager, as well as support from other members of the multidisciplinary team.

You will be HCPC registered with excellent communication and organisation skills. Proven post graduate experience in Neurological rehabilitation is essential for this role.  Hydrotherapy experience is essential for the Band 6 position.

Experienced Therapy Assistant Practitioner

Bagshot Park is a new, purpose built, state of the art Centre offering rehabilitation and to adults with an acquired brain injury and other neurological conditions. Bagshot Park specialises in Neuro Rehabilitation and acquired brain injury, being well resourced and equipped to allow an expert multi-disciplinary team to provide an optimum rehabilitation service.

We are looking for motivated and enthusiastic staff to join our team, to deliver a high standard of care to our patients.  You will possess excellent communication skills and work effectively within our multi‑disciplinary team.  You will take leadership when required to ensure smooth day to day running of the rehabilitation activities.

ESSENTIAL REQUIREMENTS

  • Minimum 2 yearsNeuro Rehabilitation Experience
  • Previous experience working as part of a wider MDT
  • Current training certificateany one of the following ( moving and handling , first aid )
  • Willingness to enrol on training and development mentorship programmes

You will be responsible for the well-being of residents within the home, ensuring that their physical, emotional and social needs are met, whilst helping them to achieve their full capabilities. In the absence of the Manager, to be responsible for ensuring the safe, smooth and efficient running of the home in accordance with the policies of the home and the Care Quality Commission guidelines and to provide leadership and be competent to support the staff.

You must have a genuine interest in providing high quality, person centred care. You must be registered with the NMC or undertaking a 20 day period of protected learning to gain NMC registration, have excellent communication skills, be motivated and professional. Main duties include clinical assessments, control and administration of medication, hands on delivery of person centred care. You will be required to give guidance, supervision and training to other staff and provide clinical support as required. Previous experience of working in a care home desirable but not essential.
